Indoor Team Dues Agreement (as applicable):

By joining WAVE's indoor team program and signing up for automatic payments, I/we (per the email address assigned to this account) am/are financially responsible for all charges incurred while the above named player is playing for WAVE Volleyball Club for the 2021-2022 club volleyball season.

I understand that all charges for the season must be satisfied per the payment schedule (regardless of choosing to either pay in full, or via installment payment plan). I acknowledge the installment payment plan option is provided as a courtesy to assist families in breaking up all costs, and dates do not correspond directly with months the athletes are in the gym for training or competition. I agree to pay all costs/fees per the team’s payment schedule. I acknowledge volleyball is a team sport, and the club’s scheduled fees are the responsibility of each family, as payments cover all shared costs for team and club participation. I acknowledge season dues are non-refundable, and are payable in full by joining the team.

By committing to WAVE and to this team, my player(s) is/are committed to attending all scheduled tournaments per the season information flier. I/we are committed to pay for any applicable tournament fees included in dues per the schedule. The team will be registered for all events listed, and all team costs are based on all team members supporting the cost to attend the events listed.

Virus considerations or other contagion-related closure, temporary or otherwise:

All dues are non-refundable, as dues cover all shared fixed costs for team and club participation (e.g., coaches, insurance, facilities). Dues are owed through virus-related closures if mandated by governing bodies. If a team has to quarantine, there will be no refunds, but WAVE will offer up makeup practices to the most reasonable extent possible.

I am financially responsible for the above player’s travel expenses including, but not limited to, room and board. In committing to playing for WAVE this season, I am responsible to pay for the season in its entirety unless unforeseen injury or illness occurs. Should an injury or illness occur, I am responsible for providing a physician’s note explaining my athlete’s length of absence and a corresponding note releasing return to play. I acknowledge receipt of the payment and playing schedules.

I acknowledge WAVE's payment requirements must be fulfilled in a timely manner per the payment schedule. If my player's account is not current, at 30 days past due I acknowledge my player will be unable to participate in further training or tournament play.

ISPE Dues Agreement (as applicable):

By joining WAVE's ISPE program and signing up for automatic payments, I/we (per the email address assigned to this account) am/are financially responsible for all charges incurred while the above named player is participating in the given semester. I understand that all charges for the semester must be satisfied per the payment schedule. I agree to pay all fees as scheduled. In committing to WAVE ISPE this semester, I am responsible to pay for the semester in its entirety unless unforeseen injury or illness occurs. Should an injury or illness occur, I am responsible for providing a physician’s note explaining my athlete’s length of absence and a corresponding note releasing return to play. I acknowledge receipt of the payment schedule.

I acknowledge WAVE's ISPE payment requirements must be fulfilled in a timely manner per the payment schedule. If my player's account is not current by semester's end, I acknowledge my player will be unable to receive their respective grade released.
